This is a FREE community course to support people living with long-term health conditions to better manage their condition and their lives.

About this Event

Living Well with a Long-Term health condition is your community-based programme which was originally developed at Stanford University.This is a group programme (max 18 participants per group) that runs for 6 weeks. Each session lasts 2.5 hours.

The course will be on Tuesday mornings from 10 to 12.30, starting on the 10th September and ending on the 15th October.

Each programme is facilitated by certified leaders.

The skills you will learn include:

  • Problem solving and decision making
  • How to set management goals
  • Managing your medication
  • Communicating effectively with your family, friends and healthcare team
  • Relaxation and managing stress
  • Healthy eating, weight management and appropriate exercise techniques
  • Managing pain, fatigue and difficult emotions

This programme is for adults 18 years and over. It is suitable for you if you are:

  • living with one or more long-term heath conditions
  • caring for someone with a long-term health condition, depression, intellectual disability or dementia
  • feeling low, anxious, unhappy or depressed

Please note: The programme is not suitable for anyone with intellectual impairment or with advanced dementia.
